1 · The report — what must it contain?

Main tournament report

Tournament name · category · dates · city · club · number of courts.
The full officiating team (all appointed officials).
Results — winners and runners-up, men's and women's.
Walkovers — each one notified and justified (score a walkover 6-0 6-0).
Facts recorded objectively / literally — no value judgment.

Warnings form — fields

Match · date · set · game-point · player · infraction · umpire.

The three annexes

Annex 1 — Organisation & equipment (venue): courts, net, lighting, balls, facilities checklist.

Annex 2 — Walkovers / warnings / incidents supplement — the less-serious stuff (code violations, WOs, minor incidents).

Annex 3 — Serious match incident → goes to the FIP Disciplinary Committee.
